How can we help you?

Tameside Adult Services offer a number of services that are focused on preventing people from becoming dependant on others for their care and support needs. Services such as Health and Wellbeing, assistive technology and reablement all help people to feel supported and safe at home and when needed other services such as home care and residential care can also be provided and will be based on an Assessment of needs and your eligibility for Social Care services.

Adult Services focuses on reducing people's need for care and support through a preventative, enabling approach where people are supported to be as independent as they can be. Our services are set up to ensure that people are encouraged to maximise self-reliance and to enable people to learn or relearn skills in order to remain as active as possible.

This portal enables you to access information and be signposted in order to manage your own care where appropriate. The portal also allows you to make contact with us online and inform us of your care requirements and request a Social Care Assessment of your needs.
   

Does your referral relate to a needs assessment?

If the request relates to a Care Act 2014 needs assessment, this should not be submitted as a safeguarding referral.
Instead, make sure to contact Tameside's Early Support and Advice Hub (ESAH) via 0161 342 5656 to request a Care Act assessment or via the portal to raise a Care Act (2014) Assessment.
Please note referrals are monitored Monday to Friday during working hours.
Monday – Wednesday 8:30am – 5:00pm
Thursday 8:30am – 4:30pm
Friday 8:30am – 4:00pm

 Any referrals submitted after working hours or on a non-working day will be followed up on the next working day.

Referral Process

Once you have considered if your referral is for safeguarding and completed the process via the portal, it will be received by the appropriate team who will ensure that it's dealt with as quickly as possible by the most appropriate enquiry officer.
The Enquiry Officer will begin the information gathering stage identifying what measures have been put in place to protect the adult and mitigate risk, determine appropriate action required and will decide as to whether Section 42 criteria apply.
